Historical Society Membership

Membership in the Historical Society and Museum for the California Department of Forestry and Fire Protection is open to all current and retired members of CAL FIRE. Membership dues are payable via Payroll or Warrant deduction authorization. Current employees should complete the PAYROLL Deduction Authorization. Retired employees should complete the WARRANT Deduction Authorization.

Instructions for completing the Payroll and/or Warrant deduction form are listed below.

All Members

Print your Last name, First name and M.I.
Fill in your Mailing Address.
Fill in your Social Security Number
Check the box indicating your Authorized Monthly Deduction. (PERS requires a minimum $2.00 monthly deduction)
Sign and Date the form. (A signature and date is required to be kept on file as long as the deduction is in place)

Retired Members

Complete all of the information on the form as directed above.
Fill in your PERS I.D. Number. (This number is found on your warrant statement, immediately above the issue date, and is needed in order to process the CALPERS form they require us to submit to begin the deduction)

Surviving Spouse or Beneficiary

Complete all of the information on the form as directed above.
Indicate your Social Security Number. (This is needed in order to process the CALPERS form they require us to submit to begin the deduction)

Mail the completed form to:

Keep and/or make a second copy of the form for your records.

Those wishing to discontinue their membership may write a letter to the Museum requesting a discontinuance of their membership deduction. Your deduction will then be stopped.
